Howard Dean was a doctor before he was governor of Vermont, a presidential candidate, or chairman of the Democratic National Committee. Now, he draws on his medical experience to argue that Medicare and Medicaid don't need radical surgery to remain solvent. And he'll draw on his 2004 presidential campaign experience to persuade the live audience at the Oct.4 Slate/Intelligence Squared U.S. debate to vote against the proposition "Grandma's benefits imperil Junior's future." (He's already proved he has the oratory prowess to rev up a crowd.) Dean, who now works as a consultant, says it's unnecessary to slash the benefits of Social Security, Medicare, and Medicaid in order to preserve the programs for future generations. I caught up with Dean earlier this week for a conversation about why America should avoid significant entitlement reform; what changes we can make to protect Social Security, Medicare, and Medicaid for posterity; and how his perspective on the health care system has changed since he stopped practicing medicine. Excerpts from the interview are below.
Slate: What question or issue will be at the core of this debate?
Howard Dean: The core question that people are interested in is, are we stealing from the next generation with our benefits structure? Of course, I'm going to be on the no side.
Slate: Can you give us a sneak peek of what you'll argue?
Dean: First of all, the Medicare problem, which is the biggest spending problem between Social Security and Medicare, is not a Medicare problem at all. It's a health care problem. The entire health care system does not control costs in any way. So the question is, how do you fix that? And the answer is, you have to fix the whole system. You can't just blame it on the beneficiaries. Everyone who uses the system is part of the problem. On Social Security, there's some minimal tweaking that needs to be done. It really is pretty solid. But I think this argument [to cut benefits] is essentially by the right wing to undo the notion of the social safety net. They're pitting generations against each other, and it's not necessary to do that. You can control the expenditures in these programs. But again, you can't focus just on these programs because it's a systemic problem. And, again, the right wing doesn't want to do anything about health care cost, but that's where the real problem is.
Slate: So how do we reform health care?
Dean: We have to stop using fee-for-service medicine. Right now, doctors get paid by the amount of things they do to people, which makes no sense at all. The more you do to people, the more you get paid, whether it does any good or not. You've gotta change that system and give people a budget for every patient. Then the physician needs to live within that budget. You should also give individuals the power to choose which plan they want to have. You have to fundamentally alter the way we pay for medicine in this country, otherwise it's going to continue to be out of control.
Slate: There have been attempts in the past through pilot programs and panels to reform Medicare, and to inspire collaboration and care coordination between hospitals and doctors. But those attempts haven't worked so well. What will be the mechanism this time around to make those kinds of changes stick?
Dean: Congress has been disgustingly irresponsible in the last two years, partly because the Republicans have decided that they're going to have it their way or no way. Probably what has to happen is [something like] this joint committee that they put together for deficit reduction: We need to take it out of the hands of Congress and just give Congress an up or down vote on a plan.
Slate: And how do we get rid of the "fee-for-service" system, or begin to encourage quality care over quantity of procedures?

Opposition to Palestinians at U.N. Threatens U.S. Business Interests (Time.com)
A U.N. vote on Palestinian statehood may be weeks or even months away, but the verdict of Egypt's public is already in - and it could mean trouble ahead for Cairo's U.S. relations with Washington, its longtime key ally and major financial backer.
Even before the popular movement that overthrew President Hosni Mubarak turned its attention to the Israeli-Palestinian conflict next door, its authors believed they had taught the Palestinians an important lesson: That when negotiations and discreet lobbying fails to bring about change, it's time to take matters into your own hands. (See photos of protests in the West Bank.)
Many Egyptians have been surprised that Israel's Prime Minister Benjamin Netanyahu has failed to seize the opportunity of the Arab Spring to negotiate the creation of a Palestinian state while the region's attentions are consumed by other issues. If Israel's occupation of Palestinian territories is not ended soon, they warn, Israel could face far tougher responses from newly confident Arabs - even among those who quietly admire Israel's democratic values.
"Netanyahu is missing a golden chance to work with Palestinians and get this problem off our chest," says Naguib Sawiris, a wealthy tycoon who ran the Orascom telecommunications giant until May, and recently founded the secular, liberal Free Egyptians political party. "He had an advantage in the past in that he had the only democratic state in the region," he says. "Tomorrow he will be facing three, four, five democratic states. And they will deal with them (Israelis) differently." A more representative political system in Egypt and Tunisia could see resurgence of Islamist parties. Sawiris, a Coptic Christian with global business interests says Israel is the one country with which he can't do business, for a simple reason: "I would lose all my business in the Middle East."
And President Barack Obama's moves to block the Palestinian statehood bid at the U.N. could raise a similar risk for U.S. companies operating in the Arab world. Obama's interventions on Israel's behalf are stirring a wave of hostility against the U.S. on the Arab street, stirring up feelings that have been on the back burner through much of the Arab Spring, and reversing some of the goodwill earned by U.S. statements and interventions in support of Arab freedom. For months visitors landing at Cairo's International Airport have been greeted by a billboard quoting Obama saying that Americans should raise their children "to be like young Egyptians." But a backlash against the U.S. is inevitable after Washington was perceived to be siding with Israel against the Palestinians, warns Hisham Fahmy, CEO of the American Chamber of Commerce in Egypt. "What form it is going to take is anybody's guess, but you know that it is going to happen."
One immediate form of protest likely to be adopted by the Egyptian public is a consumer boycott of U.S. products. Ziad Aly, CEO and founder of Alzwad, a mobile application company in Cairo, remembers how Egyptians shunned American products during the Palestinian intifada in the early 2000s, foregoing items such as Coca-Cola, which is a staple for many Cairenes in normal times.
"Whenever America takes a big stand towards what happens in Israel, immediately you get this kind of reaction," Aly said Friday, speaking by phone from an expo for start-up companies in Alexandria sponsored by the U.S. Agency for International Development. Ten years after the Intifada, Cairo is now full of brand-name American products, from Guess jeans to Starbucks cappucinos. Several Egyptians told TIME they will now avoid buying those items, particularly if there is a local alternative readily at hand. Starbucks, for example, has faced stiff competition from Egypt's local Cilantro coffee shops since the U.S. chain opened in Cairo two years, in part, says one local business owner, because of anti-Israeli feeling. But there is more at stake for both Egyptians and Americans than Coke and Starbucks. Anti-American feeling over the Palestinian issue may well have an impact on the choices made by Egyptian voters in parliamentary elections later this year, and in presidential elections expected early in 2012 - to the benefit of those politicians and parties more willing to stand up to Israel and distance themselves from Washington, such as those aligned with the Muslim Brotherhood. Despite Mubarak's close alliance with Washington, the revolution was almost entirely devoid of anti-American slogans. "We were really happy all through the revolution that there was never an anti-U.S. or anti-Western feeling," says Aly, adding that when he traveled to Silicon Valley earlier this month, top executives at Google, Twitter and venture capital firms, all wanted to meet him, eager to discuss the Egyptian revolution with a true-live activist. He believes that enthusiasm was possible partly because the revolution seemed so separated from the fraught issues of Israeli-Palestinian relations.
But the Middle East conflict is back in focus on Egypt's streets, where the mood is less celebratory and more confrontational over the pace and form of the political transition. In May, demonstrators hung an effigy in the square with a Star of David on its chest to mark the anniversary of Israel's 1948 War of Independence, and the military fought running battles with protesters who tried to swarm the Israeli embassy in downtown Cairo. On Sept. 11, tens of thousands of protesters stormed the embassy, prompting the evacuation of the Israeli ambassador and his staff. Three protester were killed in the clashes with military and the police, and days later, Egypt's ruling Supreme Council of the Armed Forces, or SCAF, reintroduced Mubarak's hated emergency laws.
Despite the darker mood towards the U.S. and Israel, no significant player in Egyptian politics - not even the Muslim Brotherhood - is ready to abandon the 1979 Camp David U.S.-brokered peace agreement between Israel and Egypt. Even if Israel manages to block the emergence of a Palestinian state, few Egyptians have any appetite for another conflict with Israel. "The cost of not honoring the Camp David agreement is so high, that nobody would risk that," Fahmy says. "We cannot afford a conflict"

World powers seek to contain Europe debt crisis
WASHINGTON?? Finance ministers, seeking to prevent another global recession, increased pressure on European countries to resolve their debt crisis by coming up with a bold rescue plan, but there were indications of further divisions ahead over what new actions to take.
Officials from the U.S. and other countries outside of Europe, concerned at the impact the crisis is having on their own economies and jittery financial markets, told their European counterparts time is running short to prevent potential domino-style defaults in Europe.
"The threat of cascading default, bank runs and catastrophic risk must be taken off the table," U.S. Treasury Secretary Timothy Geithner told his colleagues Saturday at the annual meeting of the International Monetary Fund. "Decisions as to how to conclusively address the region's problems cannot wait until the crisis gets more severe."
He said European governments needed to join with the European Central Bank to provide stronger support to calm market fears and not work at cross purposes.
Mark Carney, the head of Canada's central bank, suggested "overwhelming" the problem by more than doubling the current euro rescue fund, increasing its size to 1 trillion euros, an amount that would equal $1.35 trillion. German Finance Minister Wolfgang Schaeuble, who leads the eurozone's largest economy, and British treasury chief George Osborne also indicated they favor boosting the rescue fund's firepower.
U.S. and global financial markets have experienced intense volatility in recent days over concerns that Greece is in danger of defaulting on its debt and that this would put further strains on major European banks that carry large Greek debt in their books.
The crisis could then drag in other heavily indebted European nations, including Portugal and Ireland, and even bigger economies such as Italy and Spain.
The IMF panel, which sets policy for the 187-nation financial institution, ended its discussions Saturday with a pledge to work decisively and in a coordinated way to deal with Europe's debt crisis.

She said it was important for the 17 governments that use the euro to meet the commitments they made in July, when they decided to give the eurozone bailout fund new pre-emptive powers and reached a deal on a second bailout for Greece
"It's implementation first and foremost," Lagarde said. "No qualification."
Greek Finance Minister Evangelos Venizelos also ruled out a debt default, saying Saturday that his country was working hard on implementing the July decisions.
"Greece is never going to default because that would have been catastrophic for the euro area and for many other countries beyond the euro area," he said in a statement.
The three days of discussions wrapped up late Saturday with a meeting of the Development Committee, which sets policy for the World Bank.
World Bank President Robert Zoellick announced at a final news conference that the World Bank planned to triple to $1.88 billion the amount of humanitarian support the bank is providing to countries in drought-ravaged areas of the Horn of Africa. The World Bank has estimated that more 13 million people in the region are in need of humanitarian assistance. Zoellick said the increased support was aimed at trying to prevent the current humanitarian crisis "should not and need not be a perpetual crisis."

ATLANTA (AP) ? Capital punishment critics are regrouping after the execution of Georgia inmate Troy Davis, trying to figure out the best way to harness the anti-death penalty sentiment the case created. Among the goals: get new like-minded people registered to vote.
"Tell them to get engaged in the political process because that's where change is going to come," said Helen Butler, executive director of the Atlanta-based Coalition For The Peoples' Agenda.
Butler was among a group of about two dozen death penalty opponents who met Thursday night in Atlanta to discuss how they could abolish capital punishment in Georgia. They are a small piece of the hundreds of thousands of people the Davis case attracted, from well-known supporters like the pope and former president Jimmy Carter to those much-less politically active.
Laura Moye of Amnesty International said she expects the Davis execution to be used to rally repeal movements across the country. She plans to meet with activists in Georgia over the next few days to plot out an attempt to banish capital punishment there.
"I'm meeting people who didn't really ever speak about the death penalty and now they are. They're hungry about the information and now they know," she said.
With Davis gone, however, the loose coalition of groups who pushed for his freedom may simply crumble.
Davis was executed late Wednesday for the 1989 murder of off-duty Savannah police officer Mark MacPhail. Defense attorneys said several key witnesses disputed their testimony and other people claimed that another man confessed to the crime, but state and federal courts repeatedly upheld the conviction.
Davis maintained his innocence even as he was strapped to a gurney in the death chamber, where he told the MacPhail family to "look deeper into this case so that you really can finally see the truth."
Prosecutors and MacPhail's relatives say they have no doubt that justice was done, but among Davis' supporters, frustration runs deep.
"We did not want to lose Troy Davis as a casualty of this war, but I do think that his execution in a real sense will only add momentum to the movement of those of us who understand that the state really cannot be trusted with the ultimate punishment," said the Rev. Raphael Warnock, who spoke on Davis' behalf at a pardons board hearing this week.
Already, there are calls for lasting changes to the capital punishment system from Davis' advocates. Former President Carter said he hopes "this tragedy will spur us as a nation toward the total rejection of capital punishment." Filmmaker Michael Moore posted a statement on his website calling for a boycott of Georgia.
The Rev. Al Sharpton, who visited Davis on death row, said he will push for a national ban on capital punishment in cases that rely on eyewitness testimony. Maryland passed such a law in 2009.
"We must not only mourn what happened to Troy Davis but take strong measures so that it does not happen again," Sharpton said.
The Davis execution comes at a time when death penalty decisions are under increased scrutiny. The number of executions has dropped by half over the last decade, from 98 in 1999 to 46 in 2010. Illinois abolished capital punishment in March and several other states, including California and Connecticut, are expected to consider similar proposals next year.
More than 3,200 U.S. inmates were on death row at the beginning of 2011, according to the Death Penalty Information Center.
Public support for capital punishment remains strong, according to several polls. This month, a CBS/NY Times poll found that 60 percent of those surveyed supported the death penalty for people convicted of murder, with 27 percent opposed and 13 percent unsure. Gallup polls over the past two decades have shown slightly higher support, though Gallup found Americans to be closely divided when asked to choose between the death penalty and life imprisonment with no chance of parole.
It's far from clear, however, whether the thousands who rallied and the hundreds of thousands who signed petitions on Davis' behalf will become any kind of political force. Organizers have announced few concrete steps, and legislative proposals have yet to take shape.
"The emotion of the moment passes and unfortunately so does the urgency to address these issues," said Bruce Barket, a New York criminal defense attorney who specializes in investigating wrongful convictions.

MONDAY, Sept. 19 (HealthDay News) -- The widely used attention-deficit/hyperactivity disorder (ADHD) medication methylphenidate -- best known as Ritalin -- appears to be tied to delayed puberty in male monkeys.
But, researchers cautioned that the finding needs to be replicated before parents should concern themselves about similar effects in boys.
Male primates given methylphenidate over a 40-month span experienced "impaired testicular descent" and smaller-sized testicles, as well as lower testosterone levels, all of which resolved over the course of the study.
"To our knowledge, it's the first observation in primates to suggest an alteration in testicular function," said study author Dr. Donald Mattison, a medical officer at the National Institute of Child Health and Human Development, part of the U.S. National Institutes of Health.
"We believe they're interesting observations, but substantially more work needs to be done . . . to make sure they're repeatable," Mattison said. "It's really too early to say there are clinical implications."
The study is published in this week's online issue of the Proceedings of the National Academy of Sciences.
The most commonly diagnosed behavioral disorder in childhood, ADHD affects between 3 percent and 5 percent of school-age kids, mostly boys. Ritalin and other forms of methylphenidate have long been the most widely used drugs to treat ADHD, which involves issues of inattentiveness, overactivity and impulsivity. Other brand name versions of methylphenidate include Concerta, Metadate and Methylin.
According to Mattison, the young rhesus monkeys in the study were given either a low dose or high dose of methylphenidate, with the low dose designed to match typical blood levels of the drug observed in human ADHD patients. The high dose was intended to produce blood levels five to 10 times higher than used in people, he said.
"Testicular descent," part of the male maturation process, was postponed "significantly" in monkeys given the high (but not lower) dose of methylphenidate, the study found, while lowered testicular volume and blood testosterone levels were noted in monkeys given either the low- or high-dose regimens.
The effects on testicular function were a complete surprise, Mattison noted, since the study was designed to evaluate other types of potential toxicity from methylphenidate, including possible DNA damage.
"I think the results are more to keep in mind, and we haven't made any recommendations for clinical practice," he said. "But we encourage clinicians and scientists to think about puberty as a potential endpoint that might be affected. So we're pretty cautious at this point."
Dr. Spyros Mezitis, an endocrinologist at Lenox Hill Hospital in New York, said hormonal systems are very similar in primates and humans, but he has not observed any testosterone-related symptoms in patients taking methylphenidate.
"This is a standalone study . . . so we need more studies to understand what's happening here," Mezitis said. "In the endocrine community, when it comes to the use of Ritalin, we're not really considering testosterone levels, but maybe we should now."
A spokesperson for Novartis Pharmaceuticals Corp., which manufactures Ritalin, said it was important to note that the study was conducted in monkeys and showed the impact of methylphenidate on puberty was not permanent.
"We have specifically examined our post-marketing data for reports of delayed sexual maturation," spokesman Michael Billings said. "From the data available, no association between the use of our methylphenidate products and delays in sexual maturation has been been established."

The laser beam combines short pulses that kick the molecules and make them vibrate, as well as long pulses that are used to "listen" and identify the different "chords." The chords include different vibrational frequencies that uniquely identify every molecule, much like a fingerprint. The high-sensitivity laser can work in tandem with cameras and allows users to scan questionable areas from a safe distance.
"The laser and the method we've developed were originally intended for microscopes, but we were able to adapt and broaden its use to demonstrate its effectiveness for standoff detection of explosives," said Dantus, who hopes to net additional funding to take this laser from the lab and into the field.
This research is funded in part by the Department of Homeland Security. BioPhotonic Solutions is a high-tech company Dantus launched in 2003 to commercialize technology invented in a spinoff from his research group at MSU.

These days there are hundreds of tests for thousands of different diseases, some that require only a vial of blood, others that include an x-ray, and those that are a bit more invasive-tiny wires inserted into blood vessels to detect problems. In testing the condition of the heart, doctors prefer to begin with the least invasive of all tests: the stress test. No, you will not be tied down to a chair and shown your piling unpaid bills, nor will you be shoved off a bridge with a bungee cord wrapped around your ankles. (Both scenarios may already be stressing you out a little.) A stress test is, for some people, even more awful than that because it involves? exercise!
A stress test gives doctors information about how a patient?s heart works during physical stress (the part where you can?t breathe and think that you are surely going to die). The test usually involves walking or running on a treadmill or pedaling on a stationary bike to make your heart beat faster. Your heart rate, blood pressure, and electrocardiogram are measured at this time. A stress test can help doctors determine if enough blood is flowing to your heart when you are active, determine your likelihood of developing heart disease, identify abnormal heart rhythms, and help you develop a safe exercise program (that will not make you feel like you might be dying from a heart attack).
There are a few different types of stress tests that can be performed, depending on how you react to each. If you are able to walk or run on the treadmill and produce a normal ECG, that may be the only test required for doctors tell the condition of your heart. If you are unable to exercise drugs can be administered to make the heart beat faster, just like if you were exercising. (Unfortunately it doesn?t help you burn any calories at all.) A stress echocardiogram (or an ?echo?) produced those wiggly lines you often see on medical shows, offering a visual of the heart?s movement under stress. The echo can reveal a lack of blood flow that other tests may not reveal. Finally, a nuclear stress test involves a small amount of radioactive substance that is injected into you. The doctor then uses a camera to see a clear picture of the heart tissue. No superpowers are promised.
There are, of course, risks associated with stress tests, though none that are too dangerous. Only one in 5,000 people will experience a heart attack during a stress test. Other risks include arrhythmia (an abnormal heartbeat), low blood pressure, and general discomfort. And if you do pass out on the treadmill during a stress test, it may not be your heart that is a problem. It could be your lungs or anemia or a general distaste for exercise. If you?ve never exercised before, a stress test is likely to be very, very stressful.

Different Types of Policies

This can also be an attractive option for policy holders who want to ensure that their surviving spouse has a stable income and is financially secure after their death.
Whole life can also be borrowed against and if the policy is cancelled for any reason, the premiums that have been invested are returned to the policy holder in payment for their investment.
Choosing a Policy
Before talking to a life insurance agent, take the time to decide for yourself what your goals are in purchasing a life insurance policy. If your goal is to provide extra needed security for your young family and potentially pay for expenses like a college education for your children or make sure that your spouse does not need to work in order to support him or herself in the case of your death, then a whole life policy might be a better choice. However, if you are looking for a relatively inexpensive life insurance policy that will pay a lump sum if it becomes necessary to make a claim, then you may be more interested in a term life policy. Be prepared, however, that an insurance salesperson will make their best attempt to sell you the type of policy for which they will make the best commission, since that is, after all, their business.

Judge orders striking Tacoma teachers back to work (AP)
TACOMA, Wash. ? A judge is ordering teachers in Washington state's third-largest school district to go back to work.
The decision came following arguments by the Tacoma School District that 19 different state courts have ruled teacher strikes illegal since 1976.
Pierce County Superior Court Judge Bryan Chushcoff said he would issue a temporary restraining order against the picketing teachers, but the language of the order wasn't expected to be decided until later Wednesday.
Hundreds of teachers began picketing in front of Tacoma's major high schools Tuesday after contract negotiations broke down over the weekend. The strike has kept 28,000 students home.
The school district said it would wait until it sees the language of the order before deciding whether classes would resume on Thursday.
